Pathway details
Find below the pathway descriptions, including their different eligibility criteria. We kindly ask you to carefully check these prior to referring. That way we avoid disappointed families, as we can't accept referrals outside of this criteria.
We can accept patients registered with any GP practice in England.

How to refer to Paloma Health?
NHS GPs in England can use our online referral form (with the exception of GPs in Norfolk and Waveney). It is quick and straightforward, typically taking only 5-10 minutes to complete. You will only be asked for essential information, in line with NICE Guideline CG128 (sections 1.2 and 1.3). If you're unable to use this online form, you can request an alternative referral template by emailing paloma.health@nhs.net using your NHSmail account.
Are you an NHS GP in the Norfolk & Waveney ICB? Please use the local N&W template referral form available in your GP IT system.
